My standard fee is $145.00 for a 55-60 minute assessment (initial appointment) and $120 per 55 minute session thereafter. You are responsible for payment of my fee at each session. I will bill to those health insurance companies that I have a contract with. In those cases, you are responsible for all co-pays and deductibles according to your plan. If I am not contracted with your insurance provider, you are responsible for full payment of my fee at the time of service. All fees may be paid in cash, check, or credit card as well as HSA cards. Checks should be made payable to Kelly Rothert, LISW, LLC.

***Note: It is your responsibility to verify coverage, including knowledge of deductible and co-pay amounts if applicable. All deductibles and co-pays are due at time of service.

24 hours notice is required to cancel or change and appointment. A cancellation fee of $50 will be charged for cancellations with less than 24 hours notice or failure to show.

Note: This office will be closed if Franklin County is under a Level 2 Snow Emergency. In the event of a Level 1 or below, sessions may be cancelled if conditions are determined to be too slick for safe travel. If you need to cancel an appointment due to a winter weather advisory in your area, or because you do not feel safe driving, the $50 late cancellation fee is waived as long as you contact the office before your appointment time. The $50 fee still applies for a no call/no show.
